Hawkins and Memorial Hermann Baptist Beaumont Hospital of a surgical error that resulted in an unsuccessful tubal ligation. Rekenya Allen became pregnant with her fourth child after the tube tying procedure despite the fact that the surgery was expressly to prevent future pregnancies.
